Links4Living is the educational and public information program funded by the Kansas Real Choices Systems Change Grant. The overall goal of the Real Choices project is to make home and community based services as accessible to individuals with long-term support needs as institutional care.
The Centers for Medicare & Medicaid Services (CMS) have awarded approximately $240 Million in Systems Change Grants for Community Living to 50 States, the District of Columbia, and two U.S. territories.
In all, 287 grants have been awarded during the five funding cycles. These grants are specifically intended to help states and others build the infrastructure that will result in effective and enduring improvements in community-integrated services and long-term support systems that enable individuals of all ages
